sperm donation is it always means of AI

totally new to the sperm donation. and i have some questions memebers may have some answers to.
is it always means of AI.
does the father ever go on the birth certificate?
what happens if the child is born with disablites , is the donor held responsible?
what happens if the mothers pass away?
any answers or extra info is much appreciated
